All BISD athletes, dance team members, cheerleaders, and marching band participants need a new physical yearly. 

The UIL physical form can be found under forms. 

If your child has asthma, diabetes, seizures, and/or an allergy requiring an EPI pen please download the required form at the page below!

Please send copies of physicals to trainer@brenhamk-12.net for the 24-25 school year.

Rank One Forms are now available for the 24-25 school year. Follow this link to go to the Rank One page and complete all 3 forms - Travel Permission & Emergency Medical Info, BISD/UIL Participation Form, and Random Student Drug Testing Consent. Rank One forms are a yearly requirement for all students wanting to participate in athletics, cheer, dance and band. 

TIPS:

When entering your student's information-

-Enter their name exactly as it appears on their official school paperwork

         -Example: John Doe-Smith or John Doe Smith

-Enter their ID number without a '0' in front.

          -Example: '12345', NOT '012345'. Initials after the ID number are not needed.
